Project overview and objectives
This project is set to transform the management of chronic diseases, with a particular focus on diabetes, by creating a broadband enabled health services network to monitor and support patient care management which will improve health outcomes and provide social and economic benefits.
It will support roll out of chronic disease management applications for regional and rural Australia and lay the foundation for an industry cluster to develop health software products with export potential. The initial focus will be on establishing a network, called CDM-Net, to connect 3000 people with Type 2 diabetes in the Barwon South Western Region of Victoria with their general practitioner, health service and diabetes nurse educators to improve compliance with best practice management of their condition.
It will also help drive participation and use of broadband networks by providing a range of high-value broadband-based services to a wide range of users, including people with chronic disease, health care providers, and selected other stakeholders in health care.
