Project overview and objectives
Hunter New England Health (HNEH) will work to improve the healthcare system's ability to care for the growing number of chronic disease sufferers across a large regional and rural area through the innovative use of broadband.
HNEH will implement a chronic disease management system that operates across communities, disease groups and providers to standardise care and improve outcomes. The project will increase the amount of facility-based telehealth services available in the Hunter New England region and link chronic disease sufferers in their homes to health monitoring and support services.
This broadband-enabled access to chronic disease care means that patients will no longer have to travel long distances regularly to access their health professionals. Patients can remain in the community and have access to the same level of care that those in major centres experience.
Through increased support for both chronic disease patients and their carers, Hunter New England Health is working to improve the quality of life for chronic disease sufferers and assist health professionals to provide more efficient, high-quality care for individuals, wherever they are in the region.
